2. This doesn't calculate a rank, it could easily be implemented by having your PHP script loop through a log file checking which has the highest, and doing it that way.
3. It will not be 1-0. If you look at images 4 + 5. They show what happens if either One or Two win.
4. If you give everything the same base rating, even as it's added. It will increase the fairness, but essentially no. It will always be unfair if you add one in the middle of things. It may be worth giving dishes a "new" tag. Then the BR set to 0. When it beats something eventually, its score will slowly climb up.

This will vary from language to language and your variable type. If you're a developer, you should be able to pull it together.
